1 - Fill out the front only of this form and hand it to the captain or official of the Club giving them a verbal report of the incident
2 - As soon as possible after the game ring the Chairman of Dicipline(Norman Poole 01257 276263) and give a verbal report of the incident
3 - To send by EMAIL - Copy this form and complete and give a written description of the incident, including any foul or abusive langusge used and the names of any witnesses and send it to Norman Poole at normanpoole@sky.com withn 3 days
4 - Complete a second copyof this form and either on the back or on a separate sheet give a wriiten decription of the incident, including any foul or abusive language used and the names of any witnesses and send it to N Poole 3 Sandridge Avenue Chorley, PR7 2PZ within 3 days
N.B. - A Level 1 offence is a formal warning made to the Captain and is only recorded on nthe Match Report card not on this form.
Two Level 2 reports within one year lead to a minimum two match suspension.
A Level 3 report leads to a disciplinary hearing by the Club. If the club's action is deemed inappropriate to the offence the League Disciplinary Chairman will take action according to the League Disciplinary Guidelines
A Level 4 report is for extreme abusive and /or violent behaviour where a player is asked to leave the field
